The Real Reason AI Agents Stall

It's rarely the model. It's the boring plumbing: permissions, state, and a human who trusts the result.

When an agent demo stalls in production, the postmortem rarely says 'the model was too dumb.' It says the agent couldn't get the right permission, lost the thread between steps, or did the task and nobody believed the output enough to act on it. The model is the easy part now.

Permissions are the silent killer. An agent that can read but not write, or writes to the wrong place, is a very smart thing with its hands tied. Most agent failures I've seen were access problems wearing a reasoning costume, solved by an admin grant, not a bigger model.

State is the second. Agents that forget what they were doing mid-task produce half-finished work that looks done. The boring infrastructure of memory and checkpoints decides whether an agent is reliable, and that infrastructure is where teams underinvest because it's not the demo.

The human-trust piece is the one nobody schedules. An agent that acts and reports later gets disabled; one that checks in at the right moment gets adopted. The real reason agents stall isn't capability, it's that we shipped the brain and skipped the guardrails, the memory, and the brake.
